What is TrueLifeCare?
TrueLifeCare offers a human connection diabetes care program. We provide nurses, bilingual nurses, who become the angel on your shoulder there in your journey to manage your disease. We provide unlimited testing supplies, a free blood pressure cuff for those who have hypertension. 93% of people who have type two also have hypertension. Our nurses also provide all the education around appropriate eating and an exercise program. Most importantly, appropriate medication use. Type two diabetes is a lifestyle disease, so medication alone is not the answer.
How is TrueLifeCare different?
As it says on the screen, we are a human connection solution. We use technology to support all we're doing, but we believe that our nurses are the angels on your shoulder there to help an individual manage the disease. We have phone calls with our members at least once a month. Some members talk to their coach several times a month. We're proactive, not reactive. Our nurses don't wait until someone's sugar levels get really high to reach out to them. Our engagement rates and retention rates exceed national averages. Engagement rates range from 35% all the way up to 57%, and retention rates average between 70 and 90%. Our goal is to decrease the medical spending for those in our program. Our data's been validated by the Validation Institute to show cost savings, and we redirect monies the company is already spending on test supplies to pay for our program. Don't overpay the pharmacy, pay us instead, and we'll provide the test supplies and all that we offer.
Who is a good fit for TrueLifeCare?
TrueLifeCare works with self-funded organizations with more than 500 employees. Any organization that wants to decrease the ER visits and hospitalizations for those who have diabetes, anyone who wants to decrease worker comp claims and the medical spending for those who have the disease. It costs four times as much to employ someone that has diabetes, so if you have 500 people that have diabetes, you're spending a lot on medical care for those that have diabetes. And lastly, anyone that cares about the health and wellbeing of their employees that have diabetes.
